Otherkin are souls that don't match up to their physical bodies. Because of this, each has their own unique anatomy and they each have physical abilities, similar-but-not-quite-so to Hollows and Mod-souls. Otherkin are hated and feared by most of the Soul Society. Because of this, many choose to live in communities of their own, in abandoned buildings or forests or other places of a closed-off nature.
There are three ways that an Otherkin soul can come into being; A natural misplacement of the soul (Natural Otherkin); A natural mutation of the soul during life (Mutant Otherkin); an unnatural mutation from outside forces (Mutate Otherkin). Some 'Natural Otherkin' may also mutate to more resemble their physical body, resulting in a Hybrid Otherkin.
Because they are seen by Soul Society as irreversibly tainted by their sins, where Hollows can be purified by the Zanpakutou, they are officially classified as Natural Hollows in Soul Society. There is no such thing as a Natural Hollow, though. It's just the higher-ups being thick-headed. Most Otherkin are not let into Soul Society and are left to become Hollows. To prevent this, most live in tribal communities and groups. Because of a strong sense of community that has been established in most of these tribes and groups the Hollow transformation is slowed considerably.
Each Otherkin has an anatomy unique to them. This gives them physical abilities that other souls do not. These abilities are usually based on the form the soul takes and usually use little to no Reiatsu. Things like telescopic limbs, retractable teeth, impenetrable skin, super strength and things like that.
The only way to know for sure that the soul is an Otherkin, besides the appearance, is the position of their Chain of Fate. Because of their unique anatomies, the Chain of Fate is never on the chest, as standard, but somewhere else on the body. The position of the Chain is usually a hint to their base personality. For example, a voluptuous-minded woman might have it on her thigh while an extremely smart Otherkin might have it on the back of their head.
The largest group of Otherkin in Japan is the Sacred Hearts tribe, located in an abandoned hospital called Sacred Hearts in Karakura Town.
